Key Responsibilities

• Conduct thorough assessments to identify the social, emotional, and psychological needs of clients, facilitating tailored interventions.
• Deliver individual and group therapy sessions utilizing evidence-based practices, such as cognitive-behavioral therapy and solution-focused therapy.
• Develop actionable service plans that align with clients' goals, tracking progress and adjusting approaches as necessary to ensure effective outcomes.
• Advocate for clients by helping them access critical resources, rights, and services that promote their well-being and address ongoing social justice issues.
• Engage in program development initiatives that enhance service delivery, contribute to grant reporting, and participate in program evaluation processes.
• Collaborate effectively with community organizations, healthcare providers, and local agencies to create a supportive network for clients.
• Participate in team meetings and training sessions to share insights, enhance your skills, and contribute to collective learning and growth.

Patient Population / Client Focus

Your primary focus will be providing support to patients in the hospice setting and their families, addressing the unique emotional and psychological needs related to end-of-life care. You will work closely with clients experiencing grief, loss, and situational crises, helping them find meaning and navigate their circumstances with dignity and respect.

Qualifications

• A Master’s degree in Social Work from an accredited institution, ensuring a strong theoretical foundation for practice.
• An active state license as a Licensed Clinical Social Worker (LCSW), validating your qualifications and ability to provide clinical services.
• Strong communication skills, with an emphasis on empathy, active listening, and the ability to build rapport with clients from diverse backgrounds.
• Demonstrated organizational skills to manage multiple caseloads effectively and adhere to regulatory and ethical standards.

Preferred Experience

• Previous experience in a non-profit or hospice setting, where direct patient support is required.
• Familiarity with trauma-informed care methodologies and a commitment to social justice advocacy.
• Experience collaborating with multidisciplinary teams to enhance overall care and support for clients.

Compensation and Benefits
