Physical medicine and rehabilitation services at Health Plaza A in Fond du Lac are located at 421 Camelot Drive. The physical medicine and rehabilitation department is designed to help patients overcome the physical and psychological challenges associated with injuries. Through comprehensive, individualized therapy, our providers will help you get back to work, play and your daily lives. We practice hands-on, evidence-based therapy to provide you with the best results.
