What we want

The Diabetes & Obesity Program Assistant provides standard program and participant support. Assists in the coordination of Diabetes & Obesity Population Health Program activities, including onsite support at program locations, outreach events, classes, workshops, and community sites.

What You Need to Know

Salary: $18.20 hourly

Location: If offered this position, your primary work location will be Hallettsville, TX, Cuero, TX, or Port Lavaca, TX . Some travel to surrounding program locations will be required.

Travel: This position may require up to 20% or more of travel.

Schedule: This position may require work beyond normal office hours and on weekends.

Responsibilities:

Patient Services Navigation

• Provides culturally appropriate, non-clinical support, navigation, and follow-up to participants and families.
• Collects and records assigned participant biometric measures, including A1c, cholesterol, blood glucose, blood pressure, BMI, body fat percentage, weight, waist circumference, and hip circumference, in accordance with training and approved protocols.
• Educate participants about program services, appointments, materials, Wellness Center resources, and tracking logs; set up follow-up expectations.
• Encourage participant engagement, goal setting, and adherence to individualized plans and provider recommendations.
• Identify barriers to participation and connect participants to appropriate healthcare, social, community, Wellness Center, or Medication Assistance Program resources.
• Assists participants with scheduling appointments, completing forms or applications, accessing services, and reviewing program logs.
• Communicates participant needs, barriers, biometric information, and concerns to relevant program or clinical staff and refers clinical questions to licensed providers or diabetes education professionals.

Program Coordination & Participant Support

• Assists with coordination of Diabetes & Obesity Population Health Project activities, including onsite support at program locations, outreach events, classes, workshops, and community sites.
• Supports referral, enrollment, onboarding, participant tracking, and non-clinical care coordination workflows.
• Serves as a program contact for participants, providers, clinic staff, Wellness Center staff, and community partners.
• Coordinates communication among CBHEC, hospital, clinic, Wellness Center, healthcare, and community partners.
• Provides appointment reminders and document participant outreach, contacts, and follow-up attempts.
• Distributes participant kits, educational materials, forms, logs, and other program resources.

Program Administration & Documentation

• Prepare program correspondence, reports, presentations, forms, participant communications, meeting materials, and other documents.
• Maintains program files, databases, referral records, enrollment records, contact logs, biometric records, kit distribution records, Wellness Center referral logs, and other documentation.
• Collects and organizes program data for quarterly reports, project evaluation, partner updates, and continuous quality improvement activities.
• Supports project monitoring by identifying operational issues, documentation gaps, workflow needs, participation challenges, and opportunities for improvement.
• Coordinate logistics for program meetings, agendas, minutes, travel, outreach events, classes, workshops, Wellness Center activities, and related events.
• Maintains inventory of participant kits, educational materials, tracking logs, supplies, and other resources.
• Supports updates to program workflows, procedures, resource lists, forms, and participant materials as directed.
• Maintains confidentiality and comply with applicable HIPAA, privacy, security, infection control, institutional, documentation, and professional standards.

Qualifications :

Required Education and Experience

• Bachelor’s degree in applicable field or equivalent combination of education and experience

Required Licenses and Certifications

• Texas Department of State Health Services (DSHS) certification, certified Community Health Worker (CHW), or able to successfully complete the CHW certification within six months of employment
• Association of Diabetes Care & Education Specialists (ADCES) Diabetes Community Care Coordinator certification, or able to successfully complete the Diabetes Community Care Coordinator certification within six months of employment

Preferred Qualifications

• Ability to speak English and Spanish
• Two years of related experience in public health, health outreach, and education
• Experience working with healthcare organizations, providers, patients, or community-based organizations.
• Current Texas Department of State Health Services (DSHS) certification, certified Community Health Worker (CHW) or Association of Diabetes Care & Education Specialists (ADCES) Diabetes Community Care Coordinator certification

• Ability to communicate clearly and effectively to ensure understanding
• Knowledge of word processing, spreadsheet, database, and standard office applications
