PATIENT NAVIGATOR - QIP - FULL-TIME DAYS (24555)

New Bridge Medical Center is looking for a Patient Navigator. Responsibilities Communicate with patient care team members (physicians, behavioral health team and/or other health care providers) to provide integrative care. Carry out tasks to execute the medical and support service plans, including guiding patients to appointments and accompanying them when necessary. Act as the primary liaison with medical providers to ensure patient adherence to care. Track patient attendance for medical appointments, initiate outreach, and adhere to missed appointment procedures. Coordinate treatment plan adherence. Monitor internal data dashboard to coordinate patient care. Identify community resources to address patients’ social determinants of health and provide referrals, following up to ensure completion. Maintain program database of community partners and referrals. Maintain documentation of all patient encounters and complete reporting requirements according to organizational and contractual requirements. Maintain strict confidentiality in accordance with state and agency policies. Attend and represent the organization at professional conferences, in-service training, and meetings with supervisor approval. Additional Duties Flexibility with covering weekend/evening shifts. Perform other duties as assigned to support the overall objectives of the department and organization. Qualifications Excellent communication and interpersonal skills. Experience working with behavioral health and/or substance use population. Experience working in a medical or social services environment preferred. Strong attention to detail. Proficient with computer databases. Analytical problem‑solving skills. Ability to manage time, prioritize, and meet deadlines. Ability to work independently and as a team member. Proficiency in Spanish preferred. Education • Bachelor’s Degree in behavioral health field required. • Master’s Degree in behavioral health field preferred. Salary & Benefits Salary commensurate with experience within posted range: $42,000–$60,000 per year.
