Compensation: 92,000-115,000 Position Highlights Providebehavioral health assessments, counseling, and therapeutic services Workcollaboratively with an interdisciplinary healthcare team Supportpatients and families through treatment, education, and care coordination Responsibilities Conductcomprehensive psychosocial and behavioral health assessments Provideindividual, family, and group counseling as appropriate Developand implement individualized treatment plans Assistpatients in identifying and accessing appropriate community resources Providecrisis intervention and supportive counseling when needed Collaboratewith physicians, nurses, behavioral health providers, and other members of the care team Coordinatereferrals and follow-up care Maintainaccurate and timely clinical documentation Provideeducation and support to patients and their families Qualifications Master’sdegree in Social Work from an accredited program Current Licensed Clinical Social Worker (LCSW) license Active Arizona license required oreligibility to obtain licensure #J-18808-Ljbffr Northeast Healthcare Recruitment
