Hospice Registered Nurse

Providence at Home with Compassus Shift and Employment: Day shift 5 days / flexible Full time Position Summary Join a team where compassionate care meets clinical excellence. We are seeking a Hospice Registered Nurse (RN) to provide person-centered care in the home environment, assess patient and family needs, and collaborate with the Interdisciplinary Group (IDG) to develop individualized plans of care. Key Responsibilities Complete assessment of patient and family to determine hospice needs. Provide a complete physical assessment and history, and deliver professional nursing care using the nursing process. Write and initiate Plan of Care in collaboration with the IDG. Regularly reevaluate patient/family/caregiver needs and revise the Plan of Care as necessary. Develop a care plan with patient/family/caregiver and IDG that establishes goals based on nursing diagnosis and incorporates palliative nursing actions. Administer medications and treatments as prescribed by the IDG-approved Plan of Care. Participate in on‑call duties as defined by procedures. Maintain productivity standards set by organization guidelines. Documentation & Information Management Complete, maintain, and submit accurate clinical notes regarding patient condition and care. Supervise ancillary personnel and delegate responsibilities when required. Participate in orientation, education, and development of team members. Engage in personal growth and professional development through continuing education and in‑service training. Education & Experience Required: Associate or Bachelor’s degree from an accredited School of Nursing. Preferred: 1 year of RN practice; Hospice CNA or LPN experience considered. Certifications & Licenses Required: Current Registered Nurse license within state of employment. Required within 30 days of hire: National Provider BLS – American Heart Association (Vendor Managed). Physical Demands & Work Environment Regular standing, walking, and manual dexterity required. Ability to lift and move up to 50 pounds. Visual acuity for close and distance vision, color and peripheral vision, depth perception. Exposure to bodily fluids, infectious diseases, and standard medical equipment. Accommodations available for disabilities. Compensation & Benefits Salary range: $40.04–$72.09 per hour. Base pay is listed; additional compensation may be available (shift differentials, standby/on‑call, overtime, premiums, extra shift incentives, bonuses).
