The LPN must be detail-oriented, compassionate, and able to handle physically demanding tasks. The LPN is an essential member of the healthcare team offering patient care and supporting higher level nurse and physicians in delivering comprehensive medical services.Job DescriptionThe Licensed Practical Nurse (LPN) functions under the supervision of an RN or Licensed Independent Practitioner (LIP) in the clinical setting, within the scope of practice dictated by the State Boards of Nursing. The LPN demonstrates professional and clinical knowledge relative to designated clinical practice setting in the delivery of direct patient care, adhering to the level of verified competency dictated by the site specific program. The LPN works as part of a multi-disciplinary team to support and communicate effectively with other licensed independent practitioners (Physicians, NP, PA) in collaboration while providing care and treatment for patients, clinical support and teaching for patient and family members, and adheres to the legal and ethical standards of nursing practice. LPN manages responsibilities of workload with accuracy, provides excellence in customer service, performs testing, medication administration and the implementation of nursing interventions and care plans to support a holistic approach to patient care. The LPN serves as patient advocate in the prevention of illness or disease. The work of the LPN may be performed in hospitals, nursing homes, assisted living facilities, outpatient clinics, or private homes. Basic Life Support (BLS) within 90 days of hire. A valid LPN license in state(s) of practice is required.Additional job responsibilities:Document patient information and referrals in Epic.Process referrals for post-discharge services.Review in-network care options with patients and families.Provide required Medicare discharge information to patients.Coordinate transportation for patients leaving the hospital.Serve as an intake coordinator for care management services.Support Integrated Care Management (ICM) RNs and Social Workers with daily operations.Education:TS - Trade School Graduate- Licensed Practical Nurse REQUIREDCertification/LicensureLicensed Practical Nurse Compact/Multistate or in VA REQUIREDBLS required within 90 days of hire.Case Management (ACM, CCM, CCCTM, CMAC, or CMGT-BC) certification is required within one year of eligibility.Experience:1 year clinical experience in acute, long-term care, home health, hospice, Skilled Nursing Facility (SNF) preferred2 years Customer Service preferredCase management experience preferredSentara Princess Anne Hospital is a 174-bed acute care hospital that provides quality clinical outcomes, experienced physicians, advanced technology, and a patient-centered approach to care in southern Virginia Beach, as well as neighboring Chesapeake and Northeastern North Carolina communities.As a recognized accredited Primary Stroke Center, and Magnet hospital for nursing excellence, the hospital specializes in orthopedic and spine care, heart, vascular, advanced imaging, gynecological, comprehensive breast care services, and family maternity with a state-of-the-art neonatal intensive care unit.
