The Child and Family Services Agency (CFSA) established the Healthy Horizons Program which includes the Healthy Horizons Assessment Center (HHAC) and the Nurse Care Management Program (NCMP). These programs are housed in the Office of Well Being's Health Service Administration. The Healthy Horizons Assessment Center is staffed by licensed certified nurse practitioners and certified medical assistants, that provide health screenings for children and youth (birth-21 years) entering, re-entering, exiting and changing placements while in foster care. The goal is to provide health screenings in one location by experienced child welfare clinicians in a timely manner in alignment with best practice standards. The HHAC monthly mean for pre-placement screenings is 25 which are defined as an unclothed physical exam before entering a new placement; monthly mean for comprehensive physical examinations is 17 defined as an Early Periodic Screening Diagnosis Treatment based examination.
Requirements (Job duties include, but are not limited to)
- Assess the current medical, psychiatric, psychological, cognitive, emotional and behavioral status and patterns of the patient.
- Recognize adverse signs and symptoms and react quickly in emergency conditions.
- Treat patients with common acute conditions, illnesses, or minor trauma within accepted protocols and/or in collaboration with a physician.
- Obtain medical historical information from patients, as applicable.
- Obtain medical historical information from biological families, as available.
- Perform rapid laboratory tests, as indicated by medical standards (EPSDT) examination findings, and the age of the patient, including but not limited to anemia, lead, strep, pregnancy, drug screen, STI, urine dipstick, and glucose monitoring.
- Provide recommendations for the treatment and/or relief of symptoms related to acute illness.
- Prescribe medications as necessary.
- Delegate administrative and clinical tasks as appropriate to the Medical Assistant.
- Document, accurately, all patient encounters, historical data and services provided according to Agency and Medicaid standards.
- Communicate with caregivers and social workers, as indicated by medical standards (EPSDT) examination findings, and the age of the child, treatment recommendations and follow-up care requirements.
- Request consultation or referral with appropriate physicians, clinics or other health care resources as indicated by the medical findings, during the pre-screening examination.
- Promote age appropriate anticipatory guidance, health maintenance, and positive health behaviors during the medical screening process to both child/youth and accompanied adult through education and counseling.
- Coordinate follow-up services, to ensure a continuum of care, for identified conditions requiring close monitoring.
- Request medical records from community physicians, heath care facilities and hospitals.
- Communicate effectively and collaborate with a diverse group of professionals for the purpose of informing the multidisciplinary child welfare team of plans/actions, for teaching/education to benefit the patient/family/caregiver.
- Communicate with child/youth patients and families in a manner that encourages the sharing of information in an effort to provide the best care for the child/youth.
- Utilize excellent writing skills for both electronic and written communication.
- Ensure compliance and standards of care and practice in accordance with all established policies, procedures, guidelines, including HIPAA used in the medical treatment facility.
- Provide care within ethical and legal boundaries.
- Complete Agency and Healthy Horizons Assessment Center orientation.
- Participate in identified Agency-Sponsored Trainings.
- Participate in multidisciplinary case reviews and Agency work planning.
- Participate in committees, functions, staff meetings and other Agency-related meetings and provide relevant and timely information to these groups.
- Participate in activities designed to enhance the delivery of health services for children/youth, CFSA staff and caregivers.
- Ensure a safe work environment by complying with employee safe work habits, infection control policies and process improvement initiatives.
- Plan, direct, and evaluate total nursing care and functions in accordance with established philosophy.
- Be responsible for supporting orientation, training, and management of Incoming Nurse Practitioners and Medical Assistants.
- Collaborates effectively with others for continuity of care; maintain satisfactory relations with other departments and nursing units; participate in all phases of education, maintenance of records.
Qualifications
- Must possess a current, permanent, full and unrestricted District of Columbia license.
- Board Certification as a Pediatric Nurse Practitioner (PNP) or a Family Nurse Practitioner (FNP).
- Basic Life Support (BLS) Certification.
- A minimum of two years pediatric practice experience.
- Bilingual – English/Spanish Preferred.
- Experienced in serving the urban/underserved population and be culturally sensitive
- Experienced in assessing the current medical, psychiatric, psychological, cognitive, emotional and behavioral status and patterns of the patient.
- Knowledge of community resources in order to provide continuity of patient care.
- Knowledge of Medicaid documentation requirements for the claiming of federal reimbursement.
