Responsibilities Provide high‑quality clinical services within the scope of practice, as defined by state laws governing nursing practice, in accordance with the plan of care, and in coordination with other members of the health‑care team, following all applicable infection control procedures. Document patient visits thoroughly and completely per regulatory and payer requirements in the electronic medical record using the agency’s electronic visit verification program (as applicable). Actively participate in the agency’s care coordination process, including timely reporting of patients’ needs and changes in condition, attending patient case conferences, and communicating effectively with the patient, family, physician, other clinicians, agency management staff, case coordinators, and community case managers (as applicable). Perform skilled care in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client. Required Qualifications Current and unrestricted LPN licensure in the state of practice. Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation. Preferred Qualifications Current CPR certification or ability to complete within 90 days of hire. Home‑care experience. Ability to manage multiple tasks simultaneously. Able to work independently. Good communication, writing, and organizational skills. Ability to work flexible hours as required to meet identified client needs. Pay

• 43,900 – $97,100 annual total cash target pay
• 25.33 – $56.02 per visit point
• 21.11 – $46.68 hourly rate

Annual total cash compensation assumes full‑time employment (40 weekly hours) at full productivity and typically follows the range above. Compensation includes earnings from per‑visit point pay and hourly pay, based on factors such as local labor markets, education, work experience, and performance. The role receives two types of compensation depending on the work performed: per‑visit point pay when conducting visits and hourly pay for non‑visit activities such as orientation. We comply with all applicable minimum wage laws.
