Clinical Supervisor

Under the general supervision of the Sr Director Provider and Specialty Services, the LICSW is responsible for working with the primary care teams to integrate behavioral health with medical treatment plans of referred patients. Through psychosocial assessment and intervention appropriate for the diagnosis, the LICSW will assist the patient with achieving an improved quality of life. The role provides crisis intervention, emotional support, and resource linkage to optimize compliance with medical treatment plans and ensure success with self‑management plans. Care Coordination Collaborates with the care team and treating clinician to develop, plan, and assist in the management of patients with behavioral health needs Coordinates case conferences with patient, family, and care team, as appropriate Coordinates with clinical, health education, community health, and other staff members in the provision of services to patients and provides referrals for services needed such as mental health and longer‑term substance recovery programs Refers patients with longer‑term mental health needs to appropriate resources, when needed Connects individuals with State and Federal programs related to medical care services and financial support programs Support Functions Monitors, evaluates, and records patient progress Shows sensitivity to diversity and the needs of patients, family members, and other personnel Adjusts to emergency situations and is flexible in scheduling to be available to patients, families, and staff when the need for interventions arises Administrative Functions Performs legal reporting for suspected child abuse, elder abuse, domestic violence, and other applicable statute related duties Utilizes the current Electronic Health Record to complete the documented Best Practice Workflows accurately Documents timely, accurate, and meaningful information appropriate for patient care and charge capture Clinical Functions Knowledge of behavioral medicine and evidence‑based treatments including SBIRT; provides counseling for patients who would benefit from SBIRT (screening, brief intervention, and referral to treatment) Provides counseling for patients who have depression, anxiety, or other insight‑oriented behavioral needs Treats patients of all ages Organizational Expectations Attendance is an essential job function; employee comes to work daily, on time, and completes the scheduled shift Maintains patient confidentiality in accordance with HIPAA Contributes to positive team performance and a culture of service excellence through effective communication, active listening, and respect Maintains appropriate interpersonal relationships with staff, patients, and visitors at all times and works as a team member with other departments Practices workplace safety daily Employee Engagement Actively participates in all hospital Service Excellence initiatives and trainings Attends and participates in a minimum of 50% of department staff meetings and signs off on review of 100% of department staff meeting minutes Completes all required annual education on or before the due date Knowledge, Skills, and Abilities Minimal physical effort required with occasional light lifting to a maximum of 25 pounds; stooping; and picking, pinching, typing, or otherwise working primarily with fingers Ability to effectively manage considerable mental stress Ability to express or exchange ideas by means of the spoken word Ability to receive detailed information through oral communication Ability to communicate effectively with co‑workers and convey information in a clear, concise, and accurate manner Familiarity with computer and other business machines Qualifications Master's degree in social work from an accredited institution At least three years of experience in clinical social work Licensed in the State of New Hampshire as a Licensed Independent Clinical Social Worker (LICSW) #J-18808-Ljbffr
