Responsibilities

As a PT in this dual-setting role, you'll support patients throughout various recovery stages, whether in the hospital or the outpatient clinic. Responsibilities include:

• Conducting patient assessments and developing individualized treatment plans
• Delivering evidence-based rehabilitation services for post-operative, orthopedic, neurological, and chronic conditions
• Treating patients in both inpatient hospital rooms and outpatient clinic spaces
• Adjusting treatment approaches based on patient progress and goals
• Educating patients and caregivers to support continued recovery at home
• Participating in discharge planning and interdisciplinary rounds
• Collaborating closely with physicians, nurses, and therapy team members
• Maintaining thorough and accurate documentation by care standards
