Care Coordinator

Job Title: Care Coordinator Location: Columbus, OH Job Type: Full-time Griswold Home Care of Franklin/ Delaware counties is a well-established, respected provider of non-medical home care services. We are currently seeking a Care Coordinator to join our team. The successful candidate will be responsible for coordinating and managing all aspects of home care services for our clients. Responsibilities Manage and coordinate the scheduling of a caseload Evaluate clients' needs both via the phone and in person at client’s home, facility or hospital to match the right caregiver with the client, both in skill level and personality On boarding of new clients and caregivers Visit clients to ensure satisfaction Focus on the quality of care and build warm client, family, and caregiver relationships Provide guidance and loving support to clients, families, and caregivers to ensure an exceptional home care experience Mandatory after hours "on call" rotation (extra $550/week when on call) Requirements Caring, empathetic, and compassionate individual Effective communicator with a pleasant phone manner Strong verbal and written skills Ability to build strong rapport with caregivers, clients and families Creative problem‑solving skills Organized, detail‑oriented multitasker Good documentation, clerical and follow‑up skills Computer proficiency Experience with WellSky software is a plus Benefits Competitive salary with on‑call pay and benefits package, including medical, dental, and vision insurance, paid time off, and a 401(k) plan #J-18808-Ljbffr
