About the Role:
Project New Generation LLC is looking for a dedicated and experienced Home Health Service Director RN to lead and elevate our home health operations in Glendale, AZ. Responsibilities:

• Oversee and manage day-to-day clinical operations of the home health agency
• Ensure compliance with state, federal, and Medicare/Medicaid regulations and standards
• Supervise, mentor, and support clinical staff including RNs, LPNs, and home health aides
• Conduct patient assessments, care plan development, and ongoing case management oversight
• Monitor and maintain quality assurance and performance improvement initiatives
• Collaborate with physicians, patients, and families to coordinate comprehensive care plans
• Manage staffing, scheduling, and clinical documentation to ensure regulatory compliance

Requirements:

• Active Arizona Registered Nurse (RN) license in good standing
• Minimum 2+ years of home health or clinical management experience
• Strong knowledge of Medicare/Medicaid home health regulations and CMS guidelines
• Experience with OASIS documentation and home health billing processes preferred
• Proven leadership, communication, and team-building skills
• CPR/BLS certification required
• Reliable transportation and valid driver's license for occasional field visits
