Primary Care Physician Opportunity
This is an exceptional opportunity for a motivated Primary Care physician to join a team within a busy, well-respected rural practice, supported by a strong referral base and the full resources of a robust health system network that is conveniently located on the campus of Ascension Calumet Hospital.
Join an established team of 1 physician and 5 APPs
Call structure: 1:7
Practice in a high-volume rural primary care clinic with a supportive, team-oriented environment within a busy and collaborative setting
On campus with Calumet Hospital which was recognized as a 2024 Top 100 Critical Access Hospital by The Chartis Center for Rural Health for outstanding performance among the nation's rural hospitals.
Employed position within a nonprofit health system
Access to support staff
Board Certified or Board Eligible
Eligible for state medical licensure and DEA registration
Provide comprehensive patient care including evaluation, diagnosis, treatment, and appropriate documentation in compliance with payer requirements
Collaborate on quality and cost improvement initiatives, with opportunities to participate in academic programs and inpatient/outpatient consultations, including call coverage
Located in the scenic Chilton, WI area. Enjoy small-town charm with easy access to Appleton, Green Bay, and outdoor recreation

What You Will Need
Licensure / Certification / Registration:
One or more of the following required:
Physician MD/DO credentialed from the Minnesota Board of Medical Practice obtained prior to hire date or job transfer date.
Physician MD/DO credentialed from the Wisconsin Medical Examining Board obtained prior to hire date or job transfer date.
Advanced Life Support preferred. American Heart Association or American Red Cross accepted.
Education:
Doctor of Medicine (MD) or Doctor of Osteopathy (DO) required.
