Key Responsibilities\n- \n
- Conduct comprehensive assessments to understand and address the unique needs of each patient while developing tailored treatment plans. \n
- Facilitate clear communication among supervisors, direct care staff, family members, and other stakeholders to monitor and report on patient progress effectively. \n
- Create and provide a structured schedule of appointments, home visits, and progress evaluations to supervisors, ensuring ongoing support and follow-up. \n
- Coordinate essential home visits and in-home observations, performing chart reviews and data analysis to enhance clinical insights. \n
- Ensure the implementation of treatment plans is effective and monitored closely, adapting strategies as necessary based on ongoing assessments. \n
- Engage actively in Interdisciplinary Team Meetings to collaborate with healthcare professionals regarding patients’ care and treatment strategies. \n
- Document monthly and quarterly progress notes that track patient development and inform treatment modifications. \n
- Organize training initiatives to certify team members on the effective implementation of programs and comprehensive documentation practices. \n
- Conduct follow-up home visits and supportive interviews to continuously assess patients' evolving needs. \n
- Maintain thorough and organized records to ensure all relevant information is accessible for all interested parties. \n
- Act as a liaison between the clinic and the broader mental health community to support program development goals. \n
- Assist in the crafting of procedural training materials to enhance overall program effectiveness and improve patient outcomes. \n
The clientele at Oaks Integrated Health Centre encompasses a diverse range of populations, including both adults and youth. As a Licensed Clinical Professional Counselor, you can expect to engage with a variety of individuals facing different mental health challenges. Qualifications\n- \n
- A Master's or Doctoral degree in Counseling, Psychology, or a related field is required. \n
- Possession of an active and unrestricted LCPC license in the state of Maryland. \n
- A minimum of two years of counseling experience is essential, preferably in a clinical context. \n
- A strong understanding of legal and ethical standards in mental health practices. \n
- Exceptional interpersonal and communication skills to effectively build rapport with a diverse clientele. \n
- Demonstrated ability to collaborate within a healthcare team to deliver cohesive patient care. \n
- Proficiency in using Electronic Medical Records (EMR) software and related technologies is required. \n

- Specialization in areas such as family therapy, couples therapy, substance misuse, or trauma treatment is highly desirable. \n

- The salary for this position ranges from $90,000 to $100,000 annually, based on experience and caseload. \n
