Job responsibilities:

• Screen for social determinants of health and incorporate findings into care plans as applicable.
• Work 32 hours per week with 8-hour shifts.
• Coordinate discharge planning activities and communicate plans with patients and care teams.
• Support patient and family meetings related to care planning and transitions as needed.
