Responsibilities

• Provide direct patient care to residents in the long-term care setting, including routine assessments and ongoing monitoring of health status.
• Administer prescribed medications and treatments, with accurate documentation and adherence to physician orders.
• Monitor vital signs, observe for changes in condition, and communicate updates to the supervising RN or physician; implement care plans as directed.
• Assist residents with activities of daily living and maintain a safe, comfortable, and dignified environment.
• Collaborate with an interdisciplinary team to plan and adjust care, document care activities, and ensure continuity of care during shifts.
• Maintain compliance with infection control, safety, privacy (HIPAA), and facility policies; demonstrate professional communication and teamwork.

Requirements

• Active, current LPN license in Alabama (or equivalent with eligibility to practice in the state)
• Graduation from an accredited practical nursing program
• 1 year of active patient care experience preferred, with LTC or skilled nursing experience preferred
• Basic Life Support (BLS) certification required
• Ability to work 3x12-hour night shifts and adapt to a long-term care environment
• Strong communication, documentation, and patient safety skills
