Job Summary:

Freedom at Home, a Combat and Service Disabled Veteran owned and operated Small Business, is seeking compassionate and dedicated Licensed Practical Nurses (LPNs) to join our growing team in Muskegon, Michigan. Responsibilities:

• Provide skilled nursing care in patients' homes under the supervision of a Registered Nurse

• Follow and carry out individualized Plans of Care established by the supervising RN

• Perform skilled nursing visits and complete accurate, timely clinical documentation

• Monitor patient status and promptly communicate changes to the supervising RN and healthcare team

• Educate patients and caregivers on medications, treatments, disease management, and self-care techniques

• Reinforce the Home Health Aide plan of care and collaborate with the interdisciplinary team to ensure quality patient outcomes

• Participate in case conferences, staff meetings, and quality improvement activities

• Promote safe, compassionate, high-quality patient care while helping patients remain safely at home

Qualifications:

• Graduate of an accredited Licensed Practical Nursing (LPN) program

• Current, unrestricted Ohio Licensed Practical Nurse license

• Current CPR certification

• Minimum of one year of LPN experience required

• Home Health experience is preferred but not required

• Valid driver's license, reliable transportation, and auto insurance
