Pharmacy Technician - Population HealthAre you passionate about helping patients improve their health outcomes? Join our Population Health team as a Pharmacy Technician and play a key role in supporting pharmacy initiatives that enhance patient care and advance payer quality programs for Health Partners members.What You'll Do:Identify eligible patients for Population Health pharmacy programs and interventions.Conduct outbound patient outreach and engagement primarily by phone.Collaborate with patients, providers, and care teams to support medication-related initiatives.Assist with medication reconciliation and collect comprehensive medication histories.Coordinate pharmacy interventions that improve quality metrics and health outcomes.Document patient interactions and maintain accurate records in compliance with organizational standards.What We're Looking For:Experience in outpatient patient outreach and telephonic patient engagement.Strong communication skills with the ability to build rapport and assist patients remotely.Self-motivated, independent worker who can effectively manage priorities and follow through on tasks.Experience obtaining medication histories and identifying and resolving medication adherence or safety concerns.Previous Medication Therapy Management (MTM) or Medication Historian experience preferred.Commitment to delivering exceptional patient service and improving population health outcomes.Position Details:This is a hybrid position. Candidates must be able to attend onsite meetings in the Columbus, Ohio area as needed, including required in-person pharmacy department meetings and team events.
