Responsibilities
- Perform initial evaluations and develop individualized plans of care for home health patients.
- Deliver physical therapy interventions focused on gait training, strengthening, range of motion, balance, and activities of daily living to improve functional independence.
- Conduct Start of Care and subsequent revisits, documenting progress and updating care plans accordingly.
- Document all assessments, treatments, and outcomes in EPIC; ensure accurate and timely OASIS data collection when required.
- Collaborate with the interdisciplinary team to coordinate care, communicate changes in patient status, and support timely discharges when goals are met.
- Ensure patient safety during therapy, provide patient and family education, and adhere to infection control and safety protocols.

Requirements
- Recent/relevant home health experience
- EPIC electronic health record experience
- OASIS data collection and documentation experience
- Start of Care and Revisit assessment experience
- Ability to work effectively in a home health setting
- Willingness to travel as part of a travel assignment and adapt to a 40-hour work week
