The ideal candidate will provide comprehensive nursing care to patients in their own homes, manage a diverse patient population, and work independently while maintaining strong communication with the care team. Qualifications Graduate of an accredited school of professional nursing, BSN or PHN preferred. Current license as a Registered Nurse in California. Minimum of one year experience in an acute care setting or equivalent experience. Licensed driver with a valid, insured vehicle and good driving record. Current CPR certification. Strong communication and interpersonal skills. Essential Functions / Areas of Accountability Assumes responsibility for nursing practice in accordance with state regulations and home‑health standards. Conducts comprehensive assessments of client status, including physical, psychosocial, and environmental factors. Completes comprehensive assessments, including OASIS data, within 48 hours of referral unless different arrangements are made. Develops or follows individualized Plans of Care that incorporate data analysis and current scientific findings. Provides skilled interventions to achieve realistic client outcomes within specified time frames. Reports changes and updates to the care plan to reflect progress toward goals. Demonstrates competency with nursing skills such as assessment, teaching, medication administration, and culturally sensitive practice. Communicates effectively in accordance with agency requirements. Reports significant changes in client status to physician and team members promptly. Maintains client records and documentation in line with agency standards. Participates in team meetings and clarifies reporting responsibilities for new assignments. Ensures confidentiality and promotes safety for clients and coworkers. Demonstrates knowledge of safety and infection‑control practices and responds to unsafe situations. Uses equipment safely and evaluates environmental safety. Initiates referrals to other health team members when delegated by the Case Manager. Participates in ongoing professional self‑development and mandatory education. Identifies needs for professional growth and seeks additional training as needed. Supervises home care team members as delegated by the RN Case Manager. Submits visit notes, communication notes, and other documentation promptly according to agency requirements.
