Care Manager- Registered Nurse
HEALTHSOURCE OF OHIO
HealthSource of Ohio seeks a compassionate Care Manager – Registered Nurse to coordinate patient-centered care within our integrated community health centers. In this role, you will perform comprehensive assessments, develop individualized care plans, and support patients in managing chronic conditions and preventive care needs. Working closely with providers, behavioral health, dental, and pharmacy teams, you will facilitate care transitions, connect patients with community resources, and track outcomes using EHR and quality metrics.
