Patient Population The patient population served can be all patients, including geriatric, adult, adolescent, pediatric, and newborn. This also includes services which affect facility staff, physicians, visitors, vendors and the general public. Position Summary Under the direction of the department Leader(s) and Charge Nurse, the Infusion R.N. Oncology Nurse independently provides holistic, individualized care using the nursing process for patients routinely seen on the unit. Demonstrates an advanced level of clinical competency, nursing judgment, teaching skills, and a commitment to improving the quality of patient care. Independently performs general and unit-specific clinical skills. Contributes to Sierra View Medical Center's Department of Cancer Treatment Services and the nursing profession in a positive manner. Must be able to work normal/scheduled working hours to include Holidays, call-backs, weeknights, weekends, and on-call. Agrees to participate, as directed, in emergencies and community disasters during scheduled and unscheduled hours. Education / Training / Experience To perform this job successfully, one (1) year of recent appropriate nursing practice (within past three (3) years) is required. Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence if required. Ability to calculate figures and amounts such as metric system proportions and percentages if required. Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Licensure / Certifications Must have and maintain current licensure as a Registered Nurse in the state of California. Current OCN or ONS Chemotherapy and Biotherapy card or obtain within six (6) months of assignment to department. Must have and maintain current BLS Card. Responsibilities and Essential Functions At the time of patient admission to the CTC, the patient will be assessed for an immediate need with appropriate interventions applied. Initiates accurate and on‑going assessment of physical and psychosocial problems and responses of patients undergoing cancer treatment. Obtains data through health history, physical examination, review of records and communication with health care providers and significant others, as appropriate. Identifies priorities for nursing actions based on patient outcomes and patient goals. Insures that supplies and equipment necessary for the care of the patient are readily available and in working order. Promotes patient/family involvement in decision‑making processes. Functions independently within nursing scope of practice as evidenced by: Independently performing patient monitoring. Independently providing patient education. Independently performing life‑saving and limb‑saving measures. Reviews physician orders and implements as soon as possible. Identifies and clarifies questionable orders and treatment regimens and obtains satisfactory resolution by effective communication with physician or other appropriate personnel. Seeks guidance when faced with nursing procedures not yet mastered. Demonstrates ability to consistently delegate activities and patient care with consideration to continuity, staff skill level and complexity of patient care needs as evidenced by patient satisfaction, staff satisfaction and timeliness of work completion. Demonstrates skill and ability to perform waived testing as per established policy and procedure. Demonstrates knowledge of role by adherence to NPSGs as it relates to area of practice and engages the patient through teaching in activities that support the NPSGs. Participates in the Joint Commission/Centers for Medicare & Medicaid Core Measure Improvement as demonstrated by complying with evidence based practice for patients with CHF, AMI, Pneumonia and those patients undergoing surgical procedures. Demonstrates proper infection prevention skills to include standardized protocols or procedures related to central lines and indwelling Foley catheters. Measures patient outcomes and attainment of patient goals as evidenced by application of Laboratory values, X‑ray reports and other objective assessment data. Participates in adherence to the Joint Commission “National Patient Safety Goals for Hospitals” as defined by scope of practice. Documents the patient’s condition and any changes in condition accurately. Documents all treatments, medication, interventions, teaching and patient responses. Documentation reflects communications with other health care professionals and outcomes of those communications. Documentation reflects compliance with established Hospital policies and procedures. Identifies the educational needs of the patient/family and provides appropriate instructions and literature. Individualizes patient teaching based on readiness/ability to learn, and available educational materials, and documents such in patient chart. Demonstrates an understanding of teaching/learning principles and incorporates into documentation. Evaluates teaching effectiveness based on patient response and revises teaching plan to accommodate patient/family comprehension. Offers explanation of treatment regimens/procedures prior to initiation of care whenever possible. Acts as a resource for the learning needs of staff as evidenced by assistance in teaching and evaluation of other personnel, as needed.
