Our mission is to provide compassionate care for individuals living with serious illnesses while supporting their loved ones who care for them.Community Healthcare of Texas is currently recruiting for an RN Case Manager.$5,000 Sign-on bonusPosition SummaryMeet with patients and families to explain the scope of hospice services, coordinate admission of patients, and ongoing plan of care.Essential FunctionsProvide professional nursing care by utilizing all elements of the nursing processAssess and evaluate the patient's status by writing and initiating a plan of care. Regularly re-evaluating patient and family/caregiver needs, and participating in revising the plan of care as necessaryDevelop a care plan that establishes goals based on nursing diagnosis and incorporates hospice nursing actions. Include the patient and the family in the planning processAdminister medications and treatments as prescribed by the physicianProvide healthcare instruction to the patient as appropriate per assessment and planAct as Case Manager when assigned by the clinical Supervisor and assume responsibility to coordinate patient care for the assigned caseloadComplete, maintain, and submit accurate and relevant clinical notes regarding the patient's condition and care given.
