Care Coordinator RN

We are seeking an experienced Care Coordinator RN to join our case management team. This role involves collaborating with healthcare professionals to ensure comprehensive patient care and effective coordination of services.

Shift Details

Shift Length: 8 hours
Schedule: Days, Every Other Weekend (EOW)
First to Float

Unit Information

This role is focused on case management, where the Care Coordinator RN will play a critical role in facilitating patient care and ensuring smooth transitions throughout the healthcare process.

Qualifications

Registered Nurse (RN) License in California
Basic Life Support (BLS) Certification (AHA)
Advanced Cardiovascular Life Support (ACLS) Certification
Minimum of 2 years of experience in Care Coordination
Prior experience with Dignity/Common Spirit is required

Key Responsibilities

The Care Coordinator RN will be responsible for:

Coordinating patient care among various healthcare providers
Assessing patient needs and developing individualized care plans
Monitoring patient progress and outcomes
Facilitating communication between patients, families, and healthcare teams
