Part Time RN Care Coordinator - Home Health

$100 per hour

2nd Family - Katy

We are currently hiring for a Registered Nurse to fulfill a Part‑Time RN Care Coordinator to join our team! The primary purposes of this role are: (1) to assess and develop a care plan for senior clients with their non‑medical home care needs, and (2) oversee the caregivers assigned to them in the performance of their non‑medical care duties for these clients. This position offers the flexibility of primarily working remotely from home, with limited in person requirements for: (i) scheduled in‑home client assessments; (ii) occasional attendance at local community and professional networking events to build our client base, and (iii) caregiver orientations. Each client will receive a minimum of 4 in‑person assessments each year. Key Accountabilities: Assessments & Care Planning: Conduct initial consultations and 90‑day reassessments, including comprehensive nursing assessments and care plans. This can include observing, assessing, and documenting symptoms of clients. Care Management: Manage the care process, attend monthly company meetings (virtual or in‑person), update clinical records according to company policies and procedures, and find solutions to family concerns to ensure the "Grandma Guarantee" is met. This will include actively monitoring patient progress. Supervision & Training of Caregivers: Communicate with and supervise all CNA caregivers, providing direction, instruction, training and mentorship. This can include guiding caregivers on in‑home care techniques specific to the client’s needs. Training on how to conduct new caregiver orientation courses may be provided so that the RN Care Coordinator can conduct these orientations when needed. Primary Point of Contact: Act as the main contact for the client and family for nursing‑related questions or concerns and documenting such communications with clients and family members (or authorized representatives) of clients. This can include educating clients and caregivers on disease medications, plan of care and individualized treatments. "On‑Duty" Availability: Available to take calls/emails from 7:00 AM to 7:00 PM for client inquiries, with a required response time of 30 minutes or less during duty hours. Communication outside these hours is limited to emergency situations only. Schedule Flexibility: While much of the work is remote, initial assessments are typically required within 24–48 hours of a request; schedule flexibility is essential to accommodate short notice requests, and you will be required to work flexibly and professionally with fellow RN Care Coordinators where necessary and applicable. Other Reasonable Tasks: Undertake additional tasks when requested reasonably in the scope of services provided by the RN Care Coordinator to ensure the successful management of client care, including responding to occasional regulator requests. Qualifications/Educational Requirements:

  1. Be a registered nurse (R.N.) with a current license in the State of Texas.
  2. Minimum of two years of nursing experience as an R.N.
  3. Ability to relate positively and favorably with clients and staff.
  4. Demonstrate knowledge and skills necessary to provide care primarily to the elderly population, and to a lesser degree, other age groups including children.
  5. Professional demeanor and appearance.
  6. Excellent oral and written skills along with documentation skills.
  7. Demonstrate organizational, practical problem solving, and leadership skills.
Preferred Criteria: Professional Networking & Referrals (Preferred): We are looking for a well‑connected professional who can help grow our community of care. Experience: Prior or current experience working in hospitals, rehabilitation facilities, skilled nursing facilities, hospice, or assisted living facilities is highly preferred. Growth: Candidates who maintain active professional connections and can refer clients in need of our non‑medical caregiving services are strongly encouraged to apply. Spanish speaking: Preferred but not required. Compensation: Case Management: $100 per case, paid on a bi‑weekly basis. This covers all aspects of managing the case. You will start with 3-5 cases and can increase as desired.
