RN Congregational and Community Nurse - Part Time

The Congregational Nurse supports the broader community by addressing disparities in health and wellness in a clinic environment. Working under general supervision, this role promotes health and wellness for the congregations and community through access to physical and spiritual care. Essential Job Function * Assists community members in finding a primary care provider in order to connect them with access to care. * Supports the health and wellness of members through personal health counseling to develop care plans. * Identifies health disparities, including transportation, housing, and food to remedy issues that are preventing members from improving personal wellness. * Educates members on disease prevention and management, medications, and the health system in order to increase knowledge and awareness. * Organize health screenings and fairs and flu clinics to support the reduction of illness in the community. * Completes screenings and assessments such as blood pressure and blood glucose monitoring to identify potential health problems for treatment. * Provides spiritual care through prayer and listening in order to support a holistic lifestyle. * Performs other duties as assigned. Education * Required: Associates Degree in Nursing * Preferred: Bachelors Degree in Nursing Experience * Required: 5 years of clinical experience in a health care setting Preferred: * Bilingual: Spanish * Occasional weekends and evenings Licensure/Certification/Listing * Required: Registered Nurse license in North Carolina or a Compact state BLS (CPR)-American Red Cross or AHA Healthcare Provider
