Hospice Patient Care Director Clinical Excellence Leader
Gentiva Hospice
Gentiva Hospice seeks a Director of Patient Care Services reporting to the AVP of Operations to lead a high performing hospice program, oversee patient care activities, and mentor clinical staff in accordance with company policies and regulatory requirements.
You will coordinate IDG meetings, prioritize visit requests based on clinical judgment, respond to referrals with compassion, and model hands-on leadership during hospice visits.
