Responsibilities Provide pharmaceutical care to patients of all ages through medication order review, preparation, dispensing, documentation, monitoring, patient counseling, and medication reconciliation. Collaborate with patients, physicians, nurses, pharmacists, and other healthcare professionals to deliver safe, effective, and fiscally responsible care. Interpret, review, and process medication orders, screen orders for appropriateness, and intervene as warranted. Provide patients and providers with therapeutic assessments and selected disease management in assigned clinical areas, working toward improved patient and practice outcomes. Support clinical activities within the pharmacy and hospital teams, contributing to overall quality and efficiency of patient care. Qualifications Degrees: Bachelor’s degree required; Doctor of Pharmacy (PharmD) preferred. Licenses & Certifications: Registered Pharmacist. Additional Qualifications: Progressive pharmaceutical care experience or Pharmacy Practice Residency preferred. Minimum Experience: No experience required.
