Lead Field Licensed Vocational Nurse (LVN)
The Lead Field Licensed Vocational Nurse (LVN) is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.
Compensation & Productivity Standards
- Competitive Pay: $35.00-$40/hour for a Full-Time (1.0 FTE) 40-hour work week.
- Points-Based Productivity Model: Compensation is aligned with a structured productivity model where clinical tasks are assigned point values based on complexity (e.g., Routine Visits = 1.0; Complex/Wound Care = 1.5+).
- Full-Time Requirements: Maintain a productivity "floor" of 25–30 points per week (typically averaging 5–6 patient visits per day). To do 20-30 visits per week, that are 30 miles from your home address and from the 3742 Tibbetts St., Riverside, CA, 92506.
- Hybrid Operational Support: In periods of low field volume, hours will be supplemented via on-site administrative support at the office or through assigned Shift Care cases to ensure a consistent 40-hour work week.
- Performance Accountability: Full-time status and benefits are maintained through consistent achievement of productivity targets; proactive coaching is provided to support nurses in meeting these clinical standards.
- Flexible Territory Scheduling: Daily productivity expectations are dynamically adjusted to account for extended travel times or high-acuity cases, ensuring a balanced and manageable workload.
- Vacation Accrual Policy: Desire Home Care provides paid vacation time to help employees maintain a healthy work-life balance. Vacation time is accrued at a rate of 1 hour for every 20 hours worked. Accruals begin on the first day of employment and are available for use following the successful completion of the introductory period.

Essential Job Functions/Responsibilities
- Provides direct patient care as defined in State's Name Nurse Practice Act.
- Implements plan of care initiated by the registered nurse.
- Provides accurate and timely documentation consistent with the plan of care.
- Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care.
- Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinical Supervisor/Nursing Supervisor.
- Uses equipment and supplies effectively and efficiently.
- Participates in personal and professional growth and development.
- Performs other duties as assigned by the registered nurse.

Position Qualifications
- Graduate of an accredited practical nurse or vocational nursing program.
- Has one year nursing experience. We prefer one-year prior professional nursing experience in Community/home health or medical/surgical experience.
- Currently licensed as a licensed practical nurse or licensed vocational nurse in the state.
- Complies with accepted professional standards and practice.
- Demonstrates good verbal and written communication and organization skills.
- Must be a licensed driver with an automobile that is insured in accordance with state and organization requirements and is in good working order.
- Possesses and maintains current BLS Certification.
- Bilingual is a must (English and Spanish)
