Home Health Registered Nurse (RN) New Port Richey, FL
A leading healthcare organization is seeking a compassionate and skilled Home Health Registered Nurse (RN) to provide high-quality, patient-centered care in the home setting. Responsibilities
- Provide hands-on nursing care to patients in their homes using the nursing process and established clinical standards and protocols
- Assess, plan, implement, and evaluate individualized patient care plans
- Oversee and coordinate the activities of the multidisciplinary team involved in patient care
- Monitor the delivery of therapeutic services through effective written and verbal communication with all disciplines
- Ensure all scheduled home visits are authorized and supported by physician orders
- Conduct weekly reviews of patient visits and communicate care plans and anticipated needs to the Patient Care Supervisor and team assistant
- Communicate schedule changes, caseload updates, and patient care concerns to the Home Care Supervisor
- Maintain accurate and timely documentation of patient assessments and care provided
- Educate patients and caregivers to promote health, independence, and positive outcomes
- Collaborate with interdisciplinary team members to ensure continuity and quality of care
Qualifications
- Graduate of an accredited School of Nursing
- Current Registered Nurse (RN) license in the state of practice
- Current Basic Life Support (BLS) certification
- Valid Florida driver's license or ability to obtain one within 30 days of hire
- Associate Degree or Diploma in Nursing required
- Bachelor of Science in Nursing (BSN) preferred
- Minimum of one year of Registered Nurse experience required
- Previous home health experience preferred
- Strong communication, organizational, and critical thinking skills
- Ability to work independently and effectively manage patient caseloads
- Must meet all applicable background screening requirements
