Licensed Social Worker

Touchstone Communities is seeking a Licensed Social Worker to support residents and families in our skilled nursing and rehabilitation communities. You will complete psychosocial assessments, lead care planning, coordinate safe discharges, and connect residents with community resources. This role partners closely with nursing, therapy, and medical teams to address emotional, social, and financial needs while honoring faith-based, person-centered values. You will advocate for residents’ rights, guide families through transitions, and help ensure dignified, goal-focused care in a homelike environment.
