Nurse Case Manager - Home Visits & Telemedicine

Your Health is seeking a Registered Nurse to serve patients throughout the Blue Ridge area as a Visiting Nurse / Nurse Case Manager. The nurse will visit patients and facilitate with appropriate providers, travel to homes and facilities, and initiate telemedicine visits to evaluate patients. This full-time role requires reliable transportation, a valid license, and adherence to all laws and policies.
