Social Services – Licensed Social Worker (Part-Time)
Skilled Nursing Facility · Lewes, DE · Part-Time, 25 Hours/Week · Immediate Start

A skilled nursing facility in Lewes, DE is seeking a compassionate, state-certified social worker to join its Social Services team on a part-time basis. This is a meaningful role for someone who enjoys supporting residents and families through every step of their care.
Required credentials
Must be a state-certified LSW or CSW
Applicants without a current LSW or CSW will not be considered

What you'll do
Complete psychosocial assessments and participate in care planning
Support residents and families with adjustment, concerns, and emotional needs
Coordinate discharge planning and connect residents with community resources
Advocate for residents' rights, dignity, and quality of life
Participate in interdisciplinary team meetings
Maintain accurate, timely documentation in line with state and federal regulations
What you bring
Active LSW or CSW certification
Long-term care or skilled nursing experience preferred
Strong communicator with great follow-through
