Job Summary:

Seeking a dedicated Physical Therapist to evaluate, develop, and implement treatment plans for patients in a Skilled Nursing Facility setting. The ideal candidate will promote recovery and improve patients mobility and functional outcomes.

Responsibilities:

• 
  Assess patients' medical histories and current physical condition to determine appropriate treatment plans.
  
  
• 
  Develop individualized therapy programs to address mobility, strength, and pain management needs.
  
  
• 
  Implement therapeutic interventions using evidence-based techniques and modalities.
  
  
• 
  Monitor patient progress and adjust treatment plans accordingly to optimize outcomes.
  
  
• 
  Document patient evaluations, treatments, and progress notes accurately and timely.
  
  
• 
  Collaborate with nursing staff and other healthcare professionals to coordinate patient care.
  
  

Qualifications:

• 
  Current state licensure as a Physical Therapist.
  
  
• 
  Possess and maintain valid CPR certification.
  
  
• 
  Demonstrate experience working with elderly or SNF populations preferred.
  
  
• 
  Exhibit strong interpersonal and communication skills.
  
  
• 
  Ability to work independently and as part of a multidisciplinary team.
  
  
• 
  Maintain compliance with all regulatory and safety standards.
