Patient care may involve evaluations, administering prescribed treatments and therapies, care planning, communication with other members of the patients multi-disciplinary team, coordination of care, health promotion and teaching and training family members.Key ResponsibilitiesProvide Physical Therapy services as prescribed by the patients physicianMaintain a full time caseload of at least 30 visits weeklyProvide a written evaluation defining Physical Therapy needs, goals and expected outcomes in conjunction with referring physicians plan of treatmentFollow policies and procedures and adheres to all applicable patient care standards, licensure regulations and state lawsDevelop a written plan of care for Physical Therapy with reasonable goals and outcomesMaintain and adjust the plan of care as needed for assigned patients according to identified Physical Therapy goals and outcomesMaintain patient and company confidentialityMaintain effective communication between staff, healthcare team members, and familyParticipate in coordination of services (including discharge planning), with health care team, physicians, patient/guardians, nurses and othersBenefitsBenefits are offered to full time employees who work 30 hours or more a week.
