Andover Manor Rehab and Nursing is seeking an experienced MDS Coordinator LPN to join our skilled nursing facility in a full-time, salaried position.

The MDS Coordinator LPN will coordinate and complete MDS assessments to ensure accurate clinical documentation and maximum reimbursement. This role works closely with residents and interdisciplinary staff to develop detailed and comprehensive assessments.

Qualifications:

• Active Massachusetts LPN license required
• MDS experience in a skilled nursing or long-term care setting
• Strong understanding of MDS coordination and completion
• Excellent organizational and communication skills
• Direct hire opportunity - not an agency position

Compensation:
$90,000 - $95,000 per year (DOE)

MDS salary increases are tied to census.

Quality Measure bonus program with earning potential up to $7,500 annually.
