Requirements

• Experience: 2 years recent acute hospital Case Management (care coordination + discharge planning)
• Travel Experience: Required
• Charting: Epic
• Locals accepted? Yes
• Will this traveler need to float between like units? Yes, based on need
  Key Responsibilities
• Care coordination, discharge planning, admission criteria
• Ordering DME, working with HMOs/IPAs, prior auths (preferred)
• Managing caseload up to 25 patients
• Floating between units as needed
