Seven Healthcare are seeking an experienced Physical Therapist for a full-time, field-based role in Orange County, FL. This position focuses on home health services, providing care across multiple agency partners and working within various EMR systems. Clinicians will have a flexible, autonomous work environment with strong administrative and scheduling support.

- Provide care across multiple home health agency environments
- Coordinate visit times directly with patients
- Complete documentation within partner agency EMRs such as Kantime, Kinnser, Axxess, and Homecare Homebase
- Collaborate with PTAs for routine visits when appropriate
- Valid Physical Therapy license
- Experience with home health settings preferred
- Familiarity with EMR systems
- Ability to provide care to geriatric patients
