GLOBAL COMMUNITIES INC in San Diego is looking for a Lead Care Navigator to manage comprehensive care for pregnant and postpartum Medi-Cal members. This full-time position focuses on culturally responsive care and addresses health-related social needs. The ideal candidate will have at least four years of experience and an undergraduate degree in a relevant field.
