Responsibilities:

• Conduct comprehensive patient assessments and develop individualized care plans
• Administer medications, treatments, and skilled nursing procedures in a home health setting
• Monitor and document patient progress, vital signs, and changes in condition
• Coordinate care with physicians, therapists, and other healthcare team members
• Educate patients and family members on diagnoses, medications, and home care techniques
• Ensure compliance with all state, federal, and agency regulations and standards of care
• Provide specialized nursing interventions as needed

Requirements:

• Active Registered Nurse (RN) license in the state of Maryland
• Previous home health, hospice, or community nursing experience preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities
• Reliable transportation and valid driver's license
• CPR/BLS certification required
• Compassionate, self-motivated, and able to work independently

Compensation: $40.00 - $50.00 per hour
