Pharmacists are involved in all aspects of patient care and have access to patient charts, labs, medications, and diagnoses. With the ability to prescribe or modify care plans via collaborative agreements, our goal is to ensure patients receive the best and most complete, cost-effective medications and care possible. Aside from traditional outpatient dispensing, pharmacists provide clinical support to medical providers via chronic refill authorization, therapeutic interchanges, and prior authorization completions. Our holistic model extends assistance to shelter, energy, weatherization, HIV and AIDS counseling, home visits, and mobile medical/dental clinics.Explore our short clips, " WE are Yakima - WE are Family" and " YVFWC - And then we grew," for a glimpse into the dedication to our communities, health, and families.Who we are looking for:A Pharmacist passionate about serving underserved communitiesA Pharmacist with experience in community or ambulatory pharmacy practicePGY-1 & PGY-2 residencies are equivalent to work experienceWhat We Offer:Compensation: $77.87/hourThis position will provide coverage between clinics in Yakima and ToppenishShifts may vary between Monday - Friday 8:00am - 6:00pmWhat You'll Do:Provide patient care services as assigned, which include but are not limited to:Drug Utilization ReviewPatient CounselingMedication Therapy ManagementPerform Collaborative Drug Therapy Management or other post-diagnostic disease state management, pursuant to a valid agreementPrescribe drug or device, as authorized by statute, according to Drug Therapy Management ProtocolsOrder, interpret, and monitor laboratory testsReceive or transfer of a prescriptionProvide direct oversight of pharmacy personnel, ensuring work is within the scope of practice and duties of their licensureEnsure the security of the pharmacy area, including the provision of adequate safeguards against theft or diversion of prescription drugsEnsure all records and inventories are maintained in accordance with state and federal lawsEnsure new prescriptions, refills, and drug orders are correctly dispensed according to the provider's authorization and YVFWC policies and proceduresMaintain active licensure and pharmacological knowledge by attending educational workshops, reviewing professional publications, establishing professional networks, and participating in professional societiesQualificationsTo be considered for this position, you must complete a Bachelor's Degree in Pharmacy and have a current Washington State Pharmacist License.A Pharm D Degree and Bilingual (English/Spanish) speaking skills are preferredDrug testing: This position requires testing for controlled substances before employment and will be subject to further testing throughout employment.Our Mission Statement:"Together we transform our communities through compassionate, individualized care, eliminating barriers to health and well-being."Our mission celebrates inclusivity.
