Location: Greeley, CO

Status: Full-time

Schedule: Monday-Friday, 8-5

Annual Pay Range: $72,675 - $85,404

Supplemental Pay: Based on position, schedule and/or availability: Paid Mileage, Shift Diffs, Stipends

Mileage and Expense Reimbursement: Sixty-two and a half cents per mile one of the highest in the industry!

Rewarding Work You Will Do:

The Occupational Therapist is responsible for the oversight of OT services ensuring the delivery of therapeutic interventions.

Essential Duties:

• Using all information sources available completes initial, periodic and annual assessments on participants' functional ability and restorative potential.
• Participates in the IDT, communicates changes in the participant's condition and assists with the Plan of Care, and assists in 24-hour participant care delivery.
• Documents in the participant record in a timely manner.
• Determines participant and family needs related to functional ability.
• Confers with participant and family to identify participant goals and expectations.
• Coordinates with the interdisciplinary team to develop a comprehensive care plan for each participant.
• Facilitates continuity in care management through communication and education with related agency providers and caregivers.
• Instructs family and/or caregivers from homecare/ home health and NF agencies for therapeutic continuity.
• Instructs caregivers (staff and informal) in proper activities and modifications to improve functions.
• Facilitates or participates in family meetings, support groups and Family Caregiver meetings as required.
• In cooperation with Physical Therapy, provides leadership within the IDT to ensure continuity and coordination of care.
• Assesses participant home environment and makes recommendations for adaptations to improve independent function and enhance safety.
• In cooperation with IDT, plans and performs therapeutic interventions designed to keep the participant in the community and enhance quality of life to the greatest extent possible.
• Provides individual therapy for participants as needed or prescribed in the care plan.
• Carries out accepted principles and practices of occupational therapy to enhance participation in activities of daily living.
• Participates in quality studies according to the quality plan and recommends studies for annual quality plan.
• Assures that Therapeutic Communication is used with all participants.
• Ensures the Director is informed of care plan changes related to activities of daily living of participants, durable medical equipment, splints or home modifications.
• Works with Director to provide orientation and in-service programs for IDT to enhance staff understanding of occupational therapy issues, meet regulatory requirements and support performance improvement.
• Coordinates with medical equipment and orthotics providers to arrange appointments and share pertinent information.
• Coordinates with home modification program for installation of equipment and other changes to support independence and safety.
• Ensures the proper ordering, labeling and maintenance of durable medical equipment and other devices.
• In cooperation with the Director of Quality Improvement, participates in the Quality Management Program to support continued performance improvement.
• Completes and ensures completion of documentation of clinical services, reviewing medical record for continuity and completeness.

What We Will Love About You:

• Minimum Education: Graduation from a college or university, with a degree in Occupational Therapy.
• Minimum Experience: One year of experience as an occupational therapist required, with two years of SNF, hospital or inpatient rehab preferable. Home health experience also preferred. Other requirements may vary by case or department needs.
• Required License: Active Colorado OT license. Current CPR certification based on individual affiliate requirements. Valid Colorado driver's license and proof of automobile insurance in accordance with state statute.

Physical Requirements:

• Ability to lift minimum of 35 lbs.
