Telephonic Nurse Practitioner - New England License

$20,000 Sign On Bonus for External Candidates Telephonic After-Hours Call – Evenings, Nights & Rotating Weekends Required Full‑Time (40 Hours) Active and unrestricted NP license is required in one of the following states: New York, New Jersey, Connecticut, Maine, New Hampshire, or Rhode Island, and the ability to obtain a license in other required locations. Optum Home & Community Care, part of the Optum family, is creating a new integrated care model that addresses an individual’s physical, mental and social needs. As a team member, you will help patients navigate the health‑care system and connect them to key support services to keep them well at home. The Telehealth Urgent Care program is a comprehensive integrated care delivery program. The National On‑Call advanced practice clinician (APC) is responsible for providing telephonic/telehealth care and direction to patients, caregivers and facility staff providing 24/7 coverage, including holidays. Scheduling Full‑time, work‑from‑home, shift coverage includes weekday, weeknights, weekends and holidays. While shift times vary, coverage is 24/7. Flexibility to cross‑cover multiple markets and teams is required. Availability: 24/7 coverage, minimum 40 hours per week Every other weekend: 8–12 hour shifts both day and night, each FT/PT employee has up to 6 weekend shifts per year for PTO Unapproved time away or unpaid time off may require additional weekend shifts Holidays: rotation basis, coverage from 5 pm the last business day to 8 am on the following day Primary Responsibilities Available on the telephonic platform to coordinate and manage care between caregivers, facilities, hospitals, primary care providers and Optum field colleagues. Use video platform when clinically indicated. Work in a secure location to protect patient privacy. Utilize EMR to provide acute care during all shifts and holiday hours. Care Delivery Deliver cost‑effective, quality care to members. Manage medical and behavioral, chronic and acute conditions collaboratively with a physician or specialty provider. Perform comprehensive assessments and document findings accurately in compliance with documentation requirements and CMS regulations. Ensure encounters are documented to support the diagnosis and all quality elements are addressed. Utilize evidence‑based practice guidelines and attend required educational and training courses. Care Coordination Coordinate care as members transition through levels and settings. Monitor member and family needs, adjusting the plan of care as conditions change. Review orders and interventions for appropriateness and effectiveness. Have advanced care planning conversations with members and families. Evaluate plan cost‑effectiveness while meeting the needs of members, families and providers to reduce high costs and unnecessary hospitalizations. Program Enhancement Behaviors Provide excellent customer service to members, families, facility staff, the interdisciplinary team and Optum staff. Communicate regularly and effectively with internal and external parties. Meet shift scheduling and attendance expectations. Demonstrate original thinking, creativity and initiative in improving methods and processes. Work independently with minimal supervision and participate in quality initiatives. Check Optum email intermittently for trainings, communications and scheduling. Required Qualifications Education: NP – Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program. Active, unrestricted license in the state of residence and in New York, New Jersey, Connecticut, Maine, New Hampshire, or Rhode Island, and ability to obtain licenses in other required locations. Ability to gain a collaborative practice agreement if applicable. For jurisdictions allowing autonomous practice, obtain approval from the licensing board within the required timeframe. Active N‑P certification through a national board: American Academy of Nurse Practitioners or the American Nurses Credentialing Center, with certification in Family, Adult, Gerontology, or Adult‑Gerontology Acute Care. Current, active DEA licensure/prescriptive authority or ability to obtain post‑hire per state regulations. Ability to work scheduled shifts per scheduling policies. Proficient computer skills, including documenting in electronic medical records. Preferred Qualifications 3+ years of clinical experience as an APC. Active, unrestricted license in additional states: New York, New Jersey, Connecticut, Maine, New Hampshire, Rhode Island. Experience meeting complex behavioral, social, or functional needs. Experience in nursing homes or acute care settings with seniors. Understanding of geriatrics, chronic illness, acute disease management, advanced illness and end‑of‑life discussions. Ability to develop and maintain positive customer relationships. Adaptability to change. Compensation Salary range: $109,500 – $164,000. Total cash compensation includes base pay and bonus.
