Key Responsibilities\n

As a Licensed Professional Counselor, your primary responsibilities will include:

• Conducting comprehensive assessments to identify patient needs and tailor individualized treatment plans.

• Regularly communicating with supervisors, clinical staff, and family members regarding patient progress and required adjustments to care.

• Creating and maintaining a schedule for appointments, home visits, and monitoring sessions to ensure continuity of care.

• Coordinating home visits and data analysis to enhance in-home therapeutic observations.

• Effectively implementing and reviewing program plans to ensure that they meet the patients' needs.

• Participating in interdisciplinary team meetings to contribute insights and collaborate on holistic care strategies.

• Documenting progress through monthly and quarterly notes to track patient advancement.

• Facilitating training sessions to ensure all staff are proficient in the program plan and documentation processes.

Qualifications\n

To be successful in this role, you should possess the following qualifications:

• A Master's or Doctoral degree in Counseling, Psychology, or a related field.
