Intake Coordinator Assistance

Responsibilities: As an Intake Coordinator, you will help our front-line team convert client inquiries into active clients. Key aspects of this role include:

1. Tracking prospects from initial contact to start of care and acting as a dedicated resource for community members in need. a. Maintains and presents data related to periods of time taken for each step of the intake process, including the start of care date. SOC documented in records system. b. Checks in with Care Coordinators when "stalls" in progress occur and escalates the process up the organizational chart to ensure needed resources are provided as needed.
2. Your mission is to provide compassionate follow-up until all possibilities have been exhausted.
3. You will also be responsible for ensuring that all necessary documents are obtained according to the referral and that client setups align with payer source requirements, including accurate program and billing codes. a. Intake Coordinator will work with Care Coordinators to determine what forms/processes the Intake Coordinators will assist with to assure a reliable and seamless client onboarding (forms/processes assisted with should be consistent across Care Coordinator and plan type). i. Intake Coordinator will assist in development of a client onboarding flow sheet and associated process.
4. b. Assure live-in caregiver and Caregiver Select situations are identified early in the process and correct documentation is initiated, carefully following the HR onboarding flow sheet and associated processes. i. EVV Attestation of Exemption Form completed for each live-in that will be on time sheets. Performed as close to start of care plan as possible. ii. Assists HR in assuring timely and seamless onboarding of Caregiver Selects.
5. Assures that information about incoming caregivers is provided to HR and HR is updated regularly with the progress of the client case to assure that caregiver onboarding is complete before start of care. (THIS IS A CRITICAL FUNCTION and must be completed with 100% compliance). a. Please note, HR must complete CAPS check at least 5 days before orientation.

The desired candidate will possess the following: Excellent communication and organizational skills. Being bilingual is a plus! High school graduate or equivalent GED with two years of Home Care experience. Able to work independently, demonstrating sound judgment. Basic computer skills plus ability to learn scheduling platform in ERSP. Typing skills: we do require that you type a speed of 60WPM or greater, 70-90 WPM is a plus. Must have a car. Be available once a month as required for on-call duty outside of regular office hours.
