Home Health RN OpportunityDesire Home Care, Inc. is seeking a compassionate and dedicated Registered Nurse (RN) to join our team as a Home Health RN. This individual will play an integral role in providing high-quality, personalized home health care services to our clients in Riverside County. The RN will be responsible for assessing, planning, implementing, and evaluating all aspects of nursing care for our patients in their homes.Compensation- Intermittent:RN Start of Care Oasis- High Tech: $140RN Start of Care Oasis: $120-125Recert/Resumption of Care: $80-100Discharge: $80RN Supervisory Visit/ Follow-Up Visit: $60-70RN IV: $100Non-Visit DC Oasis: $40-45Benefits:Daily pay: Access your earned wages the same day you work!Medical, dental, vision401K with 4% matchPaid time offPaid sick timeReferral payPaid trainingsFirst time home buyer program assistanceSign on bonus: Complete 100 visits within 90 days & receive a $500 bonusResponsibilities:Conduct in-home assessments of patients' physical, emotional, and social needsDevelop individualized care plans based on assessment findings and collaborate with the patient, their family, and other healthcare team membersProvide skilled nursing care including wound care, medication management, and administration, and patient educationMonitor and evaluate patient's progress and adjust care plans accordinglyMaintain accurate and timely documentation of all patient care activitiesCommunicate effectively and regularly with patients, their families, and other healthcare team membersWork in compliance with all state, federal, and agency regulations and standardsParticipate in ongoing education and training to maintain clinical skills and knowledgeRequirements:Valid and current RN license in the state of CaliforniaMinimum of 1 year of recent experience in home health or related healthcare settingDemonstrated knowledge and proficiency in nursing assessment, care planning, and implementationExcellent communication and interpersonal skillsAbility to work independently and as part of a teamHave experience in pediatrics and/or geriatricsStrong organizational and time management skillsMust have a valid CA driver's license and reliable transportationMust have current BLS certificatePhysical and TB clearance valid within the yearAbility to complete documentation via agency EMR systemWork Schedule: Require a minimum of 5-10 visits per weekSelf-determinedEEOC Statement: Desire Home Care, Inc. is an equal opportunity employer and does not discriminate on the basis of race, color, religion, gender, sexual orientation, national origin, age, disability, or any other legally protected status.
