Hospice RN Case Manager

St. Mary's Home Care & Hospice is seeking a full time Hospice RN Case Manager. $10K Sign on bonus offered. Signing bonus is not applicable to internal employees, former employees who have resigned in the last year or applicants sourced from outside firms.

Hospice RN Case Manager position summary: Responsible for case managing a team of hospice patients. RN case manager will collaborate with interdisciplinary team and attend weekly Interdisciplinary meetings. RN Case manager will interact with the primary care physician and/or hospice medical director as needed to effectively manage patient symptoms. Position is full time primarily Monday-Friday. No on call or weekends.

Minimum qualifications:

• Graduate of an approved nursing education program
• Licensure as a Registered Nurse in the State of Georgia
• 2 years experience as a professional nurse in acute care, home care or ER experience required
• Home care experience preferred but not required
• EPIC experience preferred
