Description Key Responsibilities: Conduct comprehensive nutritional assessments for residents upon admission and periodically thereafter Develop, implement, and monitor individualized nutrition care plans Collaborate with physicians, nursing staff, and interdisciplinary teams Ensure compliance with federal, state, and local regulations Monitor food service operations for quality, safety, and nutritional adequacy Provide nutrition education and counseling to residents, families, and staff Document assessments and progress notes in accordance with facility policies Participate in care plan meetings and quality improvement initiatives Qualifications: Experience in long-term care or clinical nutrition preferred Knowledge of geriatric nutrition and medical nutrition therapy Strong communication and organizational skillsi> Familiarity with regulatory surveys and compliance standards Ability to work independently and as part of a multidisciplinary team 16 hours per week, $44.87- $56.10 per hour SALMON Health and Retirement offers a generous tuition reimbursement program for nursing-related programs and other degree programs relevant to your role.
