Primary Care Physician

Primary Care Physician (PCP) Responsibilities The PCP independently provides care for patients with acute and chronic illnesses encountered in the older adult patient. The PCP will take full accountability for patient care and outcomes and will appropriately seek consultation from specialists when needed, while staying involved in the detailed care of the patient. It is expected that the PCP will engage with the hospitalist whenever one of their patients is in the hospital, regardless of whether the hospitalist works for ChenMed or not. The PCP is responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with acute and complex chronic health needs. The PCP leads their care team consisting of a care promoter (medical assistant), care facilitator, and care coordinator for patients able to come to the office. For patients that are unable to come to the office—in hospital, SNF, LTC, or homebound—the PCP will engage with the transitional care team and others including case managers, acute and transitional‑care physicians, and other resources that may be available depending on the market. PCP will have an active role in the management of their center and will help cover for other providers who may be out for various reasons. It is also expected that each PCP will take an active role as needed in recruiting patients for the center and additional providers for the company. Supervises, collaborates with, participates with, or functions within a practice or collaborative agreement with an Advanced Practice Practitioner (APP) and remains accountable for the actions of the APP while employed with the company. Performs other duties as assigned and modified at manager’s discretion. Knowledge, Skills & Abilities Availability and accessibility: build trust by being open and available for walk‑in visits and answering phone calls and messages in a timely manner. Service orientation: provide care that one would want for a family member or for oneself at every interaction. Evidence based medicine: keep updated on evidence‑based medicine, and on clinical, nutritional, and lifestyle‑based interventions that improve outcomes. Physician leadership: continuously develop and improve leadership skills for the benefit of patients, team, center, and company. Quality: work with care teams toward achieving high quality outcomes and quality measures, and continuously improve practice and operations. Influence: competently and compassionately influence patients, teams, and oneself to achieve the best outcomes. Self‑care: maintain physical, mental, social, and spiritual well‑being to better care for patients. Proficient with Microsoft Office Suite and other productivity software. Willingness to travel locally as needed, nationally for initial training (2 weeks), and occasionally regionally and nationally for recruiting or training. Fluency in English, verbal and written; additional language fluency may be required for some locations. Exercise independent judgment. Pay Range $221,141 – $315,915 Salary. Final compensation will depend on experience, education, geographic location, and other relevant factors. The position may also be eligible for bonuses or commissions.
